We will at Our Discression replace or refund the item(s).

We accept the following methods of payment: Visa, Mastercard, Discover, American Express.

All prices are listed in US Dollars, unless otherwise indicated.

All applicable state, local, federal and international taxes and fees apply and will be clearly displayed during checkout.

Orders are not binding until payment as been accepted and verified.

Shipping and handling charges apply to all orders, unless otherwise specified.

We do not ship on major US holidays.

All delivery dates are estimates and are not guaranteed.

If you are not satisfied with your order for any reason, you may return it for a full refund within 90 days of receipt.

Items must be returned in the condition in which they arrived and in their original packaging.

To ensure you receive important emails, please add orders@5forms.com to your email address book or white list. In order to ensure timely production and minimize resources we will try to contact you by email, and automated phone messages regarding proofs. If we do not receive a response within 10 business days your order will ship without approval. If there is an error on your order and you DID NOT RESPOND TO THE PROOF EMAIL, you will be charged $15.00 per incorrect item to replace. Please make sure you entered a valid email address that you check often.

If you request a proof and approve the proof with an error, we may at our discretion charge you for your replacement.

Our artists and designers take the time to make sure that your imprint is entered as you provided it. If you do not take the time to view your proof and make sure that all spelling and phone/license numbers are correct you have wasted the artists time that they could have been working on someone else's design. YOU ARE RESPONSIBLE FOR CHECKING YOUR PROOF!


For Replacements: You are responsible for return shipping. If you are issued a replacement and do not return the product within 30 days, you are authorizing 5Forms to charge your card for the additional/replacement product.

For Refunds: Upon Request You will be issued pre-paid return labels which will be deducted from your total refund.
If you request a refund and do not return the product within 30 days your request will be cancelled. Pre-Paid shipping labels are parcel post so that you will receive the lowest USPS rate.
You have the option to ship the item using your own method.
If you send any product or correspondence, please make sure it is trackable, and insured.


If you pay for an item with a credit/debit card and:

  • The Purchase is Not Fraudulent
  • You Request a Replacement and receive it

And you then file a chargeback with the card issuer, we reserve the right to charge a fee no greater than $40.00 for the research, paperwork and processing associated with the rebuttal to the chargeback. You are responsible for this fee and if the order and fee are not paid within 90 days of notification by mail, your account will be sent to collections and or credit reporting bureaus.

We reserve the right to Re-Bill your original card used for the fee. Your acceptance of this is indicated by your agreeing to our terms and conditions during checkout.
