5FORMS LLC Return & Refund Policy

At 5Forms.com, customer satisfaction is our priority. We take great care to ensure your order is accurate and meets your expectations. Please review our return and refund guidelines carefully, as many of our products are custom-printed and produced specifically for your business.


Custom & Personalized Products

  • Due to the custom nature of imprinted products, we typically do not accept returns or issue refunds once production has begun, except in cases of:

    • Defective products

    • Errors caused by 5Forms.com (e.g., incorrect imprint, wrong product shipped)

    • Discretionary situations See Refunds & Replacements Section

  • We are not responsible for errors in submitted artwork, text, or order details that were approved by the customer during the proofing process.


Proof Approvals & Automatic Shipments

  • Customers receive up to 3 complimentary proofs before production.

  • If no response is received within 10 business days, orders will automatically ship without approval.

  • Automatic shipments are considered approved by the customer and are typically not eligible for return or refund unless defective.


Stock (Non-Imprinted) Items

  • Stock products that do not require imprinting may be eligible for return within 30 days of receipt, provided they are:

    • Unused and in original packaging

    • Returned with prior authorization (RMA) from our Customer Service team

  • Shipping fees are non-refundable, and customers are responsible for all return shipping costs unless the product is defective or shipped in error.


Refunds & Replacements

  • Refunds are issued in the original form of payment after returned items are received and inspected.

  • Refunds are not provided for:

    • Delays caused by carriers (UPS/USPS) or customs (for Canadian orders)

    • Orders shipped to an incorrect address provided by the customer

  • If the product is defective or an error was made by 5Forms.com, a replacement order will be produced and shipped at no additional cost.

  • Replacements not due to defects or a 5Forms error:

    • If customer refuses a replacement:

      We may require return of the original product. Customer is responsible for all return shipping charges. At our discretion, 5Forms may not issue a refund until a return is received.

      We will, at our discretion, provide a refund or credit of 50% of the original purchase price for products with errors due to customer entry, customer failure to review original order confirmation and notify us prior to production, and/or errors on customer-supplied electronic files.

    • Returned orders may be subject to a 20% restocking fee.

    • At our discretion we will provide a replacement for products with errors due to customer entry, customer failure to review original order confirmation and notify us prior to production, and/or errors on customer-supplied electronic files. Final responsibility for the quality of all customer-supplied electronic files rests with the customer.


CREDIT/DEBIT CARD TERMS:

If you pay for an item with a credit/debit card and:

  • The Purchase is Not Fraudulent

  • You Request a Replacement and receive it

  • And you then file a chargeback with the card issuer, we reserve the right to charge a fee no greater than $100.00 for the research, paperwork and processing associated with the rebuttal to the chargeback. You are responsible for this fee and if the order and fee are not paid within 90 days of notification by mail, your account will be sent to collections and or credit reporting bureaus.


Rush Production & Expedited Shipping

  • Rush Production Fees are non-refundable, even if the shipping method chosen does not meet your delivery expectation.

  • Please remember: Rush Production applies to production time only and does not include expedited shipping unless purchased separately.

 


How to Request a Return or Refund

  1. Contact our Customer Service team within 7 business days of receiving your order.

  2. Provide your order number, description of the issue, and supporting documentation (e.g., photos of product defects).

  3. Our team will review your request and provide instructions for return or replacement if eligible.
