We offer a 100% Satisfaction Guarantee on most products. We will at Our Discretion replace or refund the item(s). We will provide full credit, replacement or refund, for products found to contain errors made by us and for standard pre-designed products that were not printed using customer-supplied electronic art files. Final responsibility for the quality of all customer-supplied electronic files rests with the customer. Returned orders are subject to a 20% restocking fee.

Orders are not binding until payment as been accepted and verified.

If you are not satisfied with your order for any reason, you may return it at your expense for a full refund within 90 days of receipt.
Items must be returned in the condition in which they arrived and in their original packaging.
PROOFS:
To ensure you receive important emails, please add orders@5forms.com to your email address book or white list. In order to ensure timely production and minimize resources we will try to contact you by email, and automated phone messages regarding proofs. If we do not receive a response within 10 business days your order will ship without approval. If there is an error on your order and you DID NOT RESPOND TO THE PROOF EMAIL, you will be charged 50% of the sales price per incorrect item to replace. It is your responsibility to enter a valid email address that you check often, and to verify all proof information is correct prior to approval.
If you request a proof and approve the proof with an error, we may at our discretion charge you for your replacement.
RETURNS/REPLACEMENTS/REFUNDS:
For Replacements: You are responsible for return shipping. At our discretion, 5Forms may not issue a replacement until a return is received.
For Refunds: Upon Request You will be issued pre-paid return labels which will be deducted from your total refund. Pre-Paid shipping labels are parcel post so that you will receive the lowest USPS rate. If you request a refund and prefer to ship the items yourself, refund will be issued when product is received. You have the option to ship the item using your own method.
If you send any product or correspondence, please make sure it is trackable, and insured.
CREDIT/DEBIT CARD TERMS:
If you pay for an item with a credit/debit card and:
- The Purchase is Not Fraudulent
- You Request a Replacement and receive it
And you then file a chargeback with the card issuer, we reserve the right to charge a fee no greater than $80.00 for the research, paperwork and processing associated with the rebuttal to the chargeback. You are responsible for this fee and if the order and fee are not paid within 90 days of notification by mail, your account will be sent to collections and or credit reporting bureaus.
We reserve the right to Re-Bill your original card used for the fee. Your acceptance of this is indicated by your agreeing to our terms and conditions during checkout.
