Payment Processing
- All payments are securely processed through Shopify payment processors
- For your safety, we do not store any payment information.
- Any disputes regarding payment or refunds must be handled directly with your credit or debit card company.
- We accept the following methods of payment: Visa, Mastercard, Discover, American Express, PayPal, Google Pay, Shop Pay, and Venmo.
- All prices are listed in US Dollars, unless otherwise indicated.
- All applicable state, local, federal and international taxes and fees apply and will be clearly displayed during checkout.
- Orders are not binding until payment has been accepted and verified.
Order Processing & Production Times
- Custom Imprinted Products: 7–10 business days for production
- Products with a Custom Logo: Additional 3–5 business days added to production time.
- Stock (Non-Imprinted) Items: Typically ship within 5–7 business days.
Proofs & Artwork
- Complimentary Proofs: Up to 3 proofs are provided at no charge.
- Additional Proofs: $20.00 per additional proof after the third.
- Exact Reorders s (no imprint changes): Will not receive a proof. If a proof is requested, a $25.00 setup and artwork fee will apply
Proof Approval Policy
- We will attempt to contact you via email and automated phone messages regarding your proof.
- If we do not receive a response within 10 business days, your order will ship without approval.
- To avoid missed communications, please add orders@5forms.com to your safe sender list or email whitelist.
- Please review our Return Policy regarding automatic approvals.
Shipping Methods
We ship products anywhere in the United States using UPS and the United States Postal Service (USPS).
- Preferred Methods: UPS Ground and UPS Mail Innovations.
- Residential Deliveries: Shipped via UPS Mail Innovations (UPS hands off to USPS for final delivery).
- Verified Business Addresses: Eligible for UPS Ground delivery
- Expedited Options: UPS Next Day Air (NDA) and UPS 2nd Day Air (SDA) are available at an additional charge .
- Example: 1,000 forms ordered with NDA → 250 (minimum) shipped NDA, 750 shipped UPS Ground.
- If ordering multiple items, please specify in the Checkout Notes which item(s) should ship expedited.
- If multiple items require expedited shipping, you must select Multiple Item Shipping or call customer service to finalize.
- Important: Only the minimum production quantity will ship expedited. The remainder of the order will ship via UPS Ground.
- Shipping and handling charges apply to all orders, unless otherwise specified.
- We do not ship on major US holidays.
- All delivery dates are estimates and are not guaranteed.
Shipping Costs
- Deluxe Products:
- Orders under $75: $5 Standard Shipping.
- Orders $75.01 and over: Free Standard Shipping (Contiguous U.S. only).
- Navitor Products: Shipping calculated at checkout (carriers may include UPS Ground, USPS Priority Mail, First Class Mail, or Parcel Post).
- Outside the Contiguous U.S.: Orders shipped to Hawaii, Alaska, Puerto Rico, Saipan, or the U.S. Virgin Islands will ship via USPS Priority Mail. Standard free shipping does not apply
Canadian Orders
- Most products can be shipped to Canada, except for financial products (checks, deposit tickets). Ground, USPS Priority Mail, First Class Mail, or Parcel Post).
- Canadian orders are subject to shipping fees displayed at checkout.
- Taxes and customs fees are the responsibility of the customer.
Rush Production vs. Expedited Shipping
- Rush Production applies to production speed only, not shipping speed.
- If you require delivery by a specific date, you must select both Rush Production (if available) and the appropriate Expedited Shipping Method.
- Rush production time begins after proof approval.
- Example: Order Monday → Proof sent Tuesday → Approved Wednesday → Rush production begins Wednesday.
- Proof creation cannot be rushed. Proofs are typically provided within 48 business hours.
